Health departments in Madison and St. Clair counties are surveying residents who may be interested in getting the COVID-19 vaccine.
By completing the vaccine notification form, people will be added to a list to receive updates and notifications for the COVID-19 vaccine, which will include information on when you are able to sign up to receive your vaccine and how to sign-up for an appointment.
St. Clair County Vaccine Notification Sign Up Form is now available. Refer to the below link, which will allow you to provide your contact information. When the vaccine is available to your group you will be notified.
